CMDsmart* Questionnaire
This online survey is intended for all patients who are suspected of having CMD, have already been diagnosed with CMD, and have perhaps already been treated but have not achieved lasting success. It is also suitable for all patients who would like a second opinion or who observe symptoms or complaints that indicate CMD. This survey could also be informative for patients suffering from tinnitus, vertigo or spinal complaints, or recurring tension or numbness of the arms and legs. Hippocratic oath
