  • CMD and the Psyche

    The relationship between CMD and the psyche is still lacking sufficient attention. The answer to what comes first is simple: BOTH!

    The increasing complexity and severity of a CMD syndrome often cause suppressed or dormant mental illnesses to flare up. For this reason, an appropriate medical history and, if necessary, further diagnosis and treatment by specialists are always essential to our treatment.

    Conversely, as stress increases, the psyche seeks out its so-called organ of success. This may or may not be a single organ; often, it is a functional group of organs, such as the digestive or the cardiovascular system. Sentences such as "This makes me sick to my stomach" or "I can't breathe" are just two examples.

    The Masticatroy Organ As The Organ Of Success

    "To bite or gnash on something" probably refers to the masticatory organ as the organ of success. Finds of ancient skulls already point to the occurrence of bruxism and teeth grinding. This is not a natural wear and tear process of tooth substance and jawbone, but rather pathological signs of wear and tear which, according to today's assessment, indicate CMD, i.e., the involvement of the masticatory organ. But what is the masticatory organ, and why is it rightly referred to as an organ?

    The masticatory organ comprises all structures and their functions that are involved in the act of chewing and swallowing. These structures, however, are the morphological structures of a highly complex functional system consisting of bones, muscles, nerves, blood vessels, skin, and mucous membranes. It is also a functional system in which the first necessary digestive process is initiated. An organ that receives touch, taste, temperature, and pain. It is an organ that activates the hormone system via the reception of sensory stimuli and stimulates their release. Let's take insulin, for example. An organ without whose help we would die of thirst and starve to death. This is valid for the macro- and micro-nutrients. An organ that secretes essential enzymes that are nowhere else secreted in the digestive system and whose restriction or absence leads to severe metabolic disorders. If this balance, the order of the individual functional units, becomes unbalanced, the consequences are drastically felt. The psyche plays an eminently important role in this balance.

    Professor Rudolf Slavicek The Master Mind Of CMD

    "It is nonsense not to obey the rules of medicine" is one of the countless quotes from the mastermind of CMD, Professor DDr. Med Univ Rudolf Slavicek, who sadly left this world on 01.01.2022. He will be particularly missed in the present time, in which the industry has taken over the role of a "sufficient, economical and two-fold" form of treatment, whose primary concern is the satisfaction of shareholders and not the of the patient's well-being. A visionary on the one hand and a top-cranked CMD specialist on the other, he has consistently recognized and treated CMD as a holistic problem. He worked alternatively and energetically when it was not yet "en vogue". He passed on a wealth of experience to his students, who were prepared to follow alternative paths. Unfortunately, patients who seriously believe that they can get their CMD under control in the long term with a simple "crunching splint for the night" are mistaken. Both diagnosis and treatment are generally comprehensive and complex, and the same applies to sustainable treatment, at the end of which a cure is possible. With tremendous gratitude and in memory of a great man!

    QuaternAx

    Initially developed for ENT specialists and neurologists, especially for diagnosing vertigo (dizziness), we have also discovered this valuable tool for the diagnosis and treatment of CMD. The bite realtion, the temporomandibular joint position, and the neuromuscular pattern of the masticatory and facial muscles have an enormous influence on the neuromuscular pattern of the trunk muscles and limbs, and also on the balance, whose sensors are located in the inner ear, the cervical spine and the forntal lobe of the cerebrum. It is assumed that around 30% of "untreatable" cases of vertigo fall into the CMD field and are, therefore, treatable. Good news for all those suffering from vertigo.

    Functional Thinking

    To understand and assess the condition and the development of CMD, it is necessary to understand the underlying function. This requires appropriate anatomical knowledge and an understanding of functional circuits. "Form follows function!". This "law of nature" statement can be observed everywhere - natural river courses, whose banks are shaped by the water's force or rocks. Motion gives a supposedly softer or weaker medium the power to change hard materials. The same applies to hard tissues like bones and cartilage. The functional power of the soft tissues, primarily the muscles, shapes the hard tissue, such as bones and cartilage. If the corresponding circulation functions correctly, you will hardly find any deforming changes in the bone. However, if the circulation is out of balance, deforming changes in the bone will become increasingly apparent. It is, therefore, presumptuous to believe that hard tissue can be forced into one position without obeying the function and functional circuits of neuromuscular systems and the associated bone.

    Fortunately, "form follows function" implies maximum regenerative power and self-healing capacity once the functional circuits have been restored. In the case of CMD, this often requires the help of functional orthodontics, which is described in a separate paragraph.

    Functional Planning

    Functional planning requires functional thinking. In addition, you need a lot of experience and good observation skills as well as skilled and experienced teachers to study from. . You should also be able to understand the needs of the patient. The individuality of the patient determines the individuality of the therapy plan, and many factors determine individuality. An unclouded relationship of trust is essential for cooperation between therapist and patient, and this is a key criterion for planning. The consent and harmonic relation between the patient and the therapist decide about a successful treatment. The skill of choosing the right tool from a wealth of possibilities will lead to the expected treatment goal and a happy patient.

    Functional Treating

    At the beginning of treatment, the highest priority is to break the cycle of suffering. This is done based on the initial examination and the first diagnostic and planning documents. What this looks like in detail varies from patient to patient and cannot be dealt with in one sentence. However, there is no room for trial and error at any time! Once the initial breakthrough has been achieved and stabilized, it is crucial to implement this success sustainably and permanently. The entire spectrum of dentistry is often used, but functional maxillofacial orthopaedics is used in almost all cases. The treatment path is often supported by colleagues and therapists from other specialist fields, who are frequently consulted for diagnostics and planning.     The MEAW technology explained in detail once again. The originator of this technique is Professor Dr. Young Ho Kim from South Korea, who also taught at Tufts University in Boston, Massachusetts, USA.

     

  •  Die MEAW- und die MOAW- Technik

    Die MEAW- Technik

    It gets its name from the curved wires, the so-called multiloops. They do not look aesthetically pleasing and are certainly not the most comfortable, but this technique allows the bone to "regrow", even if the actual growth has long been completed. This technique also eliminates the need for surgery in the case of extreme malocclusions, which would normally require a combined orthopaedic-surgical solution. The treatment of CMD often requires a more extensive correction of the bite position, which can be successfully achieved using this technique.

    Die MOAW-Technik

    This technique is intended for the upper jaw. It is used when the segment is to be moved forward from canine to canine at the beginning of treatment. This technique is also suitable for adolescents when discussing tooth extraction to create space. This typically involves the extraction of the small molars, commonly referred to as the premolars. Basically, it's a no-go! Because "treating" a "lack of space", especially in an adolescent, by reducing the functional space is a contradiction in terms. The negative consequences often only become apparent in adulthood, often in the development of CMD.
